The Difference Between the Injectables For Weight Loss: Ozempic, Wegovy, Mounjaro, and Others

Let's delve into the fascinating world of three injectable medications that are revolutionizing weight loss: Liraglutide (Victoza, Saxenda), Semaglutide (Ozempic, Wegovy), and Tirzepatide (Mounjaro, Zepbound). What makes these medications truly unique is their role as GLP-1 (glucagon-like peptide-1) agonists and, in the case of Tirzepatide, a dual action as both a GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) agonist. These compounds are hormones found within the stomach and intestines, influencing how our bodies process food and regulate appetite through the activation of GLP-1 receptors.



Exploring the Intricacies of Each Medication


1. Liraglutide (Victoza, Saxenda)


Originally crafted for diabetes treatment in 2010 as Victoza and subsequently repurposed for weight loss as Saxenda in 2014, Liraglutide serves as a daily GLP-1 agonist. Recognized for its user-friendly nature and a consistent effect throughout the day, it allows for gradual dose adjustments to reach a maximum within a month. Quick resolution of potential side effects is a significant advantage, but the trade-off is the requirement for a daily injection and slightly less overall weight loss compared to other options. Victoza comes in doses of 0.6 mg, 1.2 mg, and 1.8 mg. Saxenda comes in doses of 0.6 mg, 1.2 mg, 1.8 mg, and 3 mg. 


Pros: Easy to use, consistent effect, quick side effect resolution.

Cons: Daily injection, slightly less overall weight loss.


2. Semaglutide (Ozempic, Wegovy)


Making its debut for diabetes in 2017 as Ozempic and later gaining approval for weight loss as Wegovy in 2021, Semaglutide is a weekly GLP-1 agonist. Ozempic has a visible needle, making it slightly easier to use than Wegovy which has a hidden needle with a triggering mechanism. This may lead to injection issues. Semaglutide tends to work better than Liraglutide long term (6-12 months)for weight loss. It’s also more desirable as it is a weekly injection rather than a daily one. However, side effects from semaglutide could persist for 1-1.5 weeks, causing prolonged discomfort. In addition, for some people the medicine wears off by day 5,6, or 7, making those days a struggle in controlling appetite. It may also take months to get to a dose that is optimal for appetite control. Ozempic comes in doses of 0.25 mg, 0.5 mg, 1 mg, 2 mg. Wegovy comes in doses of 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg. 


Pros: Once-weekly injection, user-friendly,  very effective in the long term.

Cons:  Potential for longer-lasting side effects. Potential for medication wearing off in the last few days of treatment week. Can take months to reach optimal dose.


3. Tirzepatide (Mounjaro, Zepbound)


The newcomer, Tirzepatide, received approval for diabetes in 2022 as Mounjaro and for weight loss as Zepbound in late 2023. Administered weekly with a maximum dose of 15 mg, it operates as a dual GLP-1 and GIP agonist. Offering a convenient weekly injection schedule and superior effectiveness for diabetes and weight management, Tirzepatide stands out. However, it comes with a concealed needle, potential for longer-lasting side effects, and takes a bit longer to reach the maximum dose (up to 6 months). The doses for Mounjaro and Zepbound are 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, 15 mg. 


Pros: Weekly injection, strong for diabetes and weight management, dual GLP-1 and GIP agonist.

Cons: Concealed needle, potential for longer-lasting side effects, longer time to reach maximum dose.
